    Please confirm that you have read, understood and are aware of the following:

    Glenbow College acknowledge that people have different dietary practices and for that reason Glenbow College will provide customized/per request meals, 3 times a day delivered to the quarantine address. If the students and the accompanies choose to cook there on food Glenbow College will do the necessary grocery shopping and delivery it to the quarantine address. The cost of the customized/per request meal plus a service fee of $50 (per day) will be paid by the student and the accompanies. The total due amount will be paid every day to Glenbow College on request.

    Access to needed toiletries, linen, cleaning supplies, other basic needs will be arranged by Glenbow College and paid by me.

    I have planned financially to support the cost of quarantine (meals, food, potential medical expenses) for 14 days in Canada prior to my academic start date at Glenbow College

    I will have access to phone and/or internet to keep in touch with Public Health if required and for check ins with my international advisor. The cost of phone and the service will be paid by me.

    I confirm that I have registered for classes and require medical insurance coverage.

    I confirm that I am entering Canada with medical insurance that provides coverage for COVID-19 during the mandatory quarantine upon entry period.

    QUARANTINE REQUIREMENTS:

    I have read, understood and I am aware of the requirement to comply with the Government of Canada’s Quarantine Act, including being aware of the penalties for violation of the Quarantine Act.

    I agree to comply with the requirements laid out in this International Student Safe Travel, Arrival and Quarantine Resource Guide and follow the self-isolation guidelines set out by the Canadian Ministry of Public Health.

    Self-isolation accommodation is for the full 14-day duration of their quarantine

    Self-isolation accommodation is not in a group or communal setting or is not near vulnerable persons (Indigenous or Seniors)

    Self-isolation transportation to their 14-day self-isolation accommodation must be using Glenbow College provided transportation to ensure no communal transportation services are used.

    Upon my arrival to my quarantine location I will contact the local Public Health (Alberta Health Services - 811) to let them know I am in mandatory quarantine, and have arrived from outside the country within the last two weeks. I will contact them immediately if I develop Covid-19 symptoms.

    A COVID 19 test is mandatory. I will arrange to have a CCOVID 19 test done no later than day 5 of quarantine and provide the results by Day 9 of quarantine to Glenbow College. You must take private transportation (taxi, not communicate with family or friends) to and from the appointment. If you do not have the resources to get there you will alert Glenbow College and we will arrange transportation. No other stops will be made on this day, only the test location and I will wear a face covering or mask the entire trip.

    I have completed and printed the Quarantine CheckList that was provided to me along with my travel letter to present at the port of entry. In addition I have registered via the ArriveCAN app.

    I confirm that I have the appropriate medical insurance, effective as of the date of my travel to Canada, which includes coverage for Covid-19 during the quarantine period

    Self-isolation accommodation is for the full 14-day duration of their quarantine. During my self-isolation for 14 days of quarantine I understand that a Glenbow College representative will daily check on me via phone. If they do not get any response from me within a reasonable time frame then Glenbow College will notify CBSA.

    It is recommended that all students arriving from outside Canada review the following mandatory guidelines for a suitable 14 day quarantine site and ensure that their plan meets these guidelines.
